If it's vaginal you definitely need to rule out a yeast infection and treat that as prescribed if necessary.
Beyond that any diaper rash cream with zinc is both soothing and gentle, or you can go to the silicon based barrier creams (Wellskin Barriere was the one recommended to us by the visiting nurse)

Most OTC lotions/creams/ointments will be gentle enough for the tender skin--but ask the pharmacist--some lotions have acids in them. Some cannot be used for long periods of time.

The OTC yeast cream often really helps with itching--but I'd take the patient in for an evaluation before trying anything, if you can. You need to be treating a rash with the proper stuff--and if they have a yeast infection--you need specific medicated cream for that.

I know that keeping the skin clean and dry is important--and that clothing doesn't chafe at the site.
