This man will defecate without washing his hands, and lie about it if you catch him emerging unwashed from the bathroom (besides their having been no sound of a sink running, he'll often leave the door open as he does his business). If asked to return to the bathroom and sanitize himself, he'll holler and curse and wish death upon me; and while he'll comply with the request at the time, he doesn't seem to develop a habit of washing his hands independently. What can I do to improve the situation?

My mother doesn't wash her hands each time she uses the bathroom, either. Really I think that she is physically and mentally tired so doesn't do things she feels aren't necessary for living.

How about putting a bottle of hand sanitizer in his bathroom and encourage him to use that instead? It's better than nothing. He's probably gone his whole life without washing his hands after using the bathroom and you can't teach an old dog new tricks.

I had that problem with my loved one too. I never got it resolved. Eventually, she became incontinent and now the staff change and clean her.

I hope you get some suggestions. It's a tough one.

Praise him when he is done, despite the curses. He will learn to look forward to the praise.
