What can be done about a rehab facility that is preventing the discharge/transfer of a patient to another facility, by purposely not returning calls or sending requested medical information to the requesting facilities?

Are you this person's PoA or guardian? Is the person in question mentally competent? If so, they are calling the shots.

The State regulates rehabs and nursing homes. Assisted living, I am sure the state comes in to inspect. Call your State Ombudsman. The phone number will be under state government.

Have you called Adult Protection Services? Tell them the situation and you need a well visit done because they will not return your calls.
