Doesn't medicare pick up the tab. If the doctor approves the order? That is what I've been told anyway................

If the patient's physician ordered Home Care after being hospitalized for an acute illness, the patient may qualify for an aide once or twice a week until the end of the episode (usually 60 days or less).  
If the patient does not qualify for a Home Care Agency services (physicians do not write home care orders arbitrarily without a sound rationale/need as they are subject to audit and need to present medical necessity for the patient to order Home Care).
Medicare/Medicaid will not pay for an aide to come in home to provide "full assistance". If the patient needs that much care perhaps they belong in a skilled nursing facility (NH).  HHA (home health aides) from an agency provide basic hygiene but usually their visits are less than an hour as they have many other clients to take care of in the course of their day. When I worked in home care there were periods where we had no available HHA due to staffing or staff vacations. 
Look to your local Dept of Aging or resources on this site  can help you find  a CNA/CG from an approved agency - but these will be private pay - family must pay out of pocket. Also do your research on whoever you choose as there are many "shady" people out there who say they are qualified but are not.
