She was ill back in Feb. and March so we had to keep her at the facility since she lives alone, she couldn't go home the way she was. Now she seems like she's back to her old self and I was wondering how do I get her back to the rehab part of the nursing home ?

Is she getting PT at all? If not, the doctor might have to order it? Was she living her prior to rehab or home? Are you considering taking her back home? If so, she could probably get home PT services. But going home could be way too much at her age. Have you talked to her to see if she's willing to do the rehab now and do the work to improve her strength, etc.? If she's willing, then push for it!

There are probably a few different answers to this question depending on several unknown factors.

The type of insurance your mom has will be important as different policies have different benefits.
Original Medicare, Medicaid. One of the Medicare Advantage plans?

Has your mom been private pay while in LTC?

I would ask the doctor to order her a PT evaluation and see where she is on being able to do therapy and then discuss how to get it for her.

You ask her doctor to order therapy and understand this starts the documentation of “making progress” all over again. My dad did this multiple times, the last being the time he truly couldn’t progress and chose to come home on hospice

You need to ask for a 24/7 evaluation. The facility will not release her if its an unsafe discharge. If found she needs 24/7 care I would leave her there.p unless u want to do the Caregiving. This means that you will need to show that her home is safe and you will be providing 24/7 care and/or hiring someone to care for her.

When Mom was in the NH she did get some PT. If you bring her home her PCP can order in home PT.

Getting her back to rehab? That would be between you and the Social Worker. If she has any of the initial 100 days and the diagnosis is the same, she may be allowed to finish out the 100 days. If she had already done her 20 that Medicare pays 100%, she will not be allowed them again. She or her suppliment will have to pay the 50% that Medicare doesn't pay.
