Just got mine. Went thru an certified elder care attorney. Took awhile but I think it will be worth it. Needed a Dr to say wife is in need of care and cannot handle her own finances and other things related to care. Court declared her incompetent and approved me a guardian for her and her estate. Here that is two guardians in one, as a guardian can be appointed separately for person and estate. I would start by checking with your local court to see what is actually involved and what may be required.

Guardianship is a surrogate court mediated proceeding. You file legal papers for guardianship and the court appoints a guardian ad litem to represent the person in question.

 The court orders evaluations and interviews and a hearing is held to determine the competence of the person in question and the fitness and suitability of the person who filed for guardianship.
